Here is a list of forms that are available for you to download.  If you do not have Acrobat Reader, please click the link below as you will need to have Acrobat Reader in order to read these files.

 

Step 1:  Application Packet (2010-2011)

All perspective students must fill out an Application Form and submit two Recommendation Forms.  There will need to be a Request for Transcript Form from a previous school, if new and accepted.  The Admission Committee will review all Application Packets to select the student roster.

Step 2:  Registration Forms

Parents will be informed by the Admission Committee when their student is selected to attend Big Horn Christian.  Once placed on the official student roster, parents must submit all Registration Forms.

(For all new students and 1st and 5th graders)

Health/Immunization Form (Pick up from local doctor, printout of their immunization record)
